Before you add a form to your website you first have to create the form. Once created, you may display your form.
Add a Form to a Ticket Category Page
You can add a form to a ticket category page by navigating to the Ticket Category section of the admin panel.
Click the edit button of the ticket category you want to add a form to (or create a new ticket category). Near the bottom of the page select Add Module > Form
Create a Title for your form (This is publicly visible), a description (optional), and select the form you want to insert.
Update the Ticket Category to save changes.
Add a Form on a Page
You can add a form to a page by utilizing the Form Module (learn how to add a module to a page here).

If you prefer, you may use a third-party for forms & embed them using the embed module. Keep in mind that without having Analytics installed on your third-party site & enabling cross-domain tracking there will be no way to measure engagement with your form.
